Iowa EMS Recertification Requirements

Iowa EMS certifications renew every two years. You can renew one of two ways: show a current NREMT certification that expires after your Iowa certification, or complete Iowa core continuing education hours (CEHs). Starting November 1, 2026, the Iowa state path requires 9 hours for EMR, 22 for EMT, 28 for AEMT, and 34 for Paramedic, including new pediatric hours at every level. The NREMT path stays at 16, 40, 50, and 60 hours.
What Changed in Iowa EMS CE on November 1, 2026
Iowa HHS adopted new EMS certification rules (ARC 0597D) on August 14, 2026. They take effect November 1, 2026 and change the Iowa state renewal path in several ways:
- New pediatric CEH requirement at every level: 1 hour for EMR, 2 for EMT, 3 for AEMT, and 4 for Paramedic.
- Higher total hours on the state path: EMR goes from 8 to 9, EMT from 20 to 22, AEMT from 25 to 28, and Paramedic from 30 to 34.
- Half-hour credit is now allowed. 50 minutes equals one CEH and 30 minutes equals one-half CEH. Courses must be at least 30 minutes long.
- Faster audit deadline. If you are audited, you have 30 days to send your documentation, down from 45.
- Shorter reactivation window. The 25 to 48 month reactivation option is gone. After 24 months inactive, you go through NREMT reentry.
The adopted rule does not spell out how a renewal cycle that started before November 1, 2026 is handled. If your cycle straddles that date, confirm which table applies to you with the Bureau before you finish your hours.
Iowa EMS Recertification: NREMT Path vs. Iowa State Path
Iowa lets you keep your state EMS certification active either by staying National Registry certified or by completing Iowa core CEHs. Here are both side by side at the hours in effect November 1, 2026.

The Iowa state path is not just a total. You need a minimum number of core CEHs in each topic area. These are the minimums under IAC 641-131.5(7) as amended, effective November 1, 2026.
Which Iowa Recertification Path Should You Choose?
Even after the November 2026 increase, the Iowa state path asks for roughly half the hours of the NREMT path at every level. So the real question is portability, not workload.
Choose the NREMT path if you might work EMS in another state, your employer requires National Registry, or you already keep NREMT current. Your NREMT expiration must fall after your Iowa expiration for it to count toward Iowa renewal.
Choose the Iowa state path if you plan to work only in Iowa and want the fewest hours. You will need to track hours by topic area, including the new pediatric hours.
What Continuing Education Counts for Iowa EMS Renewal
Iowa core CEHs must carry a sponsor number from one of these sources, and the sponsor number must be assigned before you take the course:
- CAPCE (Commission on Accreditation for Prehospital Continuing Education)
- An authorized Iowa EMS training program
- Iowa HHS (the department)
- The Iowa Board of Nursing or Board of Medicine
A few more rules worth knowing:
- One CEH equals 50 minutes. From November 1, 2026, 30 minutes equals one-half CEH and multi-hour courses earn credit in half-hour steps.
- Out-of-state CE counts if it meets Iowa's sponsor requirements and was approved for EMS personnel in the state where it was held.
- College courses in human health topics can be approved in advance: one semester credit equals 15 CEHs and one quarter credit equals 10 CEHs.
- Keep your certificates for four years after the end of each certification period. Random audits happen every cycle.
How to Renew Your Iowa EMS Certification in AMANDA
Iowa EMS renewals are filed online through the AMANDA licensing portal run by Iowa HHS.
- Finish your CE first. Complete your NREMT recertification or your Iowa core CEHs before your expiration date.
- Log in to AMANDA using Google Chrome or Microsoft Edge. The Renew option appears on your My Programs page once you are within 90 days of expiration.
- Pick your renewal method. Answer "Yes" to renewing with a current NREMT registration and attach a copy, or answer "No" and enter your total core CEHs.
- Complete the affirmation questions and attachments. Do not click Cancel, which voids the whole application.
- Pay any fee. EMR and EMT renewals have no fee. AEMT and Paramedic applications are not considered submitted until payment is made.
- Watch your email. Once approved, the Bureau activates your certification and emails your wallet card.

Late Renewal, Extensions, and Reactivation
Need more time? Request an extension
If you can't finish your CE before your certification expires, you can apply for an extension. The request and fee must reach the Bureau at least 7 days before your expiration date. CE completed during an extension can't be reused for your next renewal period.
Late renewal
If you finished your CE during the certification period but missed the renewal deadline, you may qualify for late renewal of your inactive certification. If you did not finish your CE during the period, you are not eligible for late renewal.
Reactivation after November 1, 2026
A certification inactive for up to 24 months can be reactivated with an approved application, an audit report showing core CEHs prorated by lapsed year, a background check, and the reactivation fee. The CEHs and background check must be completed after the department approves your application. After 24 months inactive, you can no longer reactivate. You would complete NREMT reentry and apply for initial Iowa certification.

Can I renew my Iowa EMS certification with NREMT?
Yes. If your NREMT certification is current and expires after your Iowa certification, you can renew Iowa by meeting NREMT requirements. You still submit the Iowa renewal application in AMANDA, attach your NREMT card, and pay any Iowa fee.
Does Iowa require NREMT certification?
Iowa requires active NREMT certification for initial state certification. For renewal, NREMT is optional. You can renew using Iowa core CEHs instead.
What EMS continuing education does Iowa accept?
